Parental Consent Form

Submit your Parental Consent Form to grant permission for photography and video recording of your child during academy activities and acknowledges the risks associated with roller skating, including potential accidents.

Instructions:

To open the Parental Consent Form, please click on the link provided. Start by selecting "Click to Start" and fill in the necessary details. In the signature area, enter the name of the person who will be signing the document, then click "draw" to create your signature and hit "sign." After all fields are completed, click "Validate & Send Completed Document." Once you submit the form, make sure to click "Download" to obtain a copy of the signed document, which you should then attach to the Signed Parental Consent Form.

Student Name

Enter and select the name of the student enrolled at Skateland.

Note-1: Certain names may begin with the parent's name, followed by a comma and the student's name.
